Choice Support has been a provider of Personal Health Budgets services since 2011, we are experienced in supporting people with a range of complex needs, helping them to maintain their independence by giving them the opportunity to have more control over the care and support services that they receive.
We work with each individual to develop a person-centred support plan ensuring that everyone’s plan is tailored to them, their needs and the outcomes they want to achieve. To ensure continuity of care, our staff will work with each individual enabling effective working relationships and trust and rapport to be developed.
