How Collaborative Care is Helping Patients Quit Smoking for Good

Each year, Stoptober provides a critical opportunity for smokers to take a meaningful step toward a healthier, smoke-free life. Research shows that those who quit smoking for the full 28 days of October are five times more likely to stay smoke-free for good.

This year, Barry’s story serves as an inspiring example of how collaborative care can truly make a difference. A lifelong smoker in his 60s, Barry was admitted to Whittington Hospital in June with pneumonia. During his stay, he opened up about his long history of smoking, revealing that he had been smoking a pack a day for years. Immediately, Nellie, the Tobacco Dependency Project Manager, along with the Whittington Stop Smoking Team, stepped in to support Barry on his quit-smoking journey.

When Barry was transferred to University College London Hospital (UCLH) for further treatment, his journey didn’t end there. Nellie made sure to coordinate with Jay from Camden’s Stop Smoking Service, ensuring Barry’s care didn’t fall through the cracks. This seamless collaboration meant Barry received consistent support across both hospitals and boroughs.

Once Barry was discharged, Nellie continued to stay in touch, offering practical guidance on the benefits of quitting and presenting him with different cessation options, such as QuickMist, lozenges, and patches. Shortly after, Jay reached out to Barry, helping him choose the right tools to support his journey. Barry opted for lozenges and patches, which were promptly delivered to his home.

Thanks to this ongoing, coordinated effort, Barry has now been smoke-free for over four months. Reflecting on his success, Barry shared: “I had considered quitting for years, but it was the stop smoking teams that gave me a clear method and the support I needed to finally quit. I am very grateful.”

Nellie underscores how vital it is to start the conversation: “By initiating dialogue about quitting, we give patients a chance to reflect on their smoking habits and offer them support they might not otherwise seek. Consistent follow-up is crucial to helping them stay on track.”

Barry’s journey highlights how collaboration between healthcare teams across Trusts and boroughs ensures patients don’t fall through the cracks and receive the best possible care.
